Massachusetts State Police are investigating a fatal crash on Route 495 in Merrimac on Sunday.
Around 11:51 a.m., troopers responded to reports of a two-car crash on Route 495.
According to police, a woman driving a 2008 Toyota Scion was traveling north on Route 495 in the left travel lane when, for reasons still under investigation, she lost control of the car and veered into the median.
The driver attempted to reenter the left travel lane but swerved across all three lanes, hitting a car traveling north in the right lane.
The Toyota went off the road and crashed into a tree.
The driver, a 23-year-old woman from Newbury, was transported to Anna Jacques Hospital where she succumbed to injuries sustained in the crash.
The driver of the Nissan was not injured.
The identity of the woman who died is not being released, pending next of kin notification.
The crash remains under investigation.
