The Middlesex District Attorney’s Office and Woburn police are investigating the death of a 15-month-old boy.
Officials say the toddler, Noah, who had been hospitalized since Monday, died Thursday night at Boston Children’s Hospital.
Sources say his skull was fractured.
His twin sister went to the hospital Tuesday. It is unclear what injuries she suffered. Sources say she needed some sort of cast.
Noah was rushed to the hospital Monday after someone called 9-1-1 from the Kilby Street apartment.
Officials say the matter is now being referred to the medical examiner’s office.
The Department of Children and Families is also a part in this investigation which is ongoing.

No charges have been filed at this time.
